Chapter 10: Dynamic Medical Certificates & Consent Form Studio
Uttercode Healthcare ERP includes a dynamic Document Template Engine (certificate_template.php) that generates legal medical certificates, informed consent declarations, and referral notes in seconds.
1. Dynamic Variable Tags
Templates use automated smart tags that replace placeholders with real patient and admission data at print time:

2. Standard Pre-Configured Templates
The system comes pre-loaded with standard hospital templates:
1. Medical Sickness / Leave Certificate
Certifies that the patient is suffering from a diagnosed illness and requires medical rest for a specified number of days (from date to date) with a declared fit-to-resume date.
2. Medical Fitness Certificate
Certifies physical examination fitness for employment, school/college admission, or general travel.
3. Surgical & Anesthesia Informed Consent Form
Standard NABH-compliant informed consent document detailing planned surgical procedure risks, blood transfusion authorization, and patient/guardian signatures.
4. High-Risk / ICU Consent Form
Special declaration for critically ill patients being admitted to the Intensive Care Unit or undergoing high-risk emergency interventions.
5. DAMA / LAMA (Discharge Against Medical Advice) Consent
Legal declaration signed by the patient or relative when leaving the hospital against doctor advice, absolving the medical institution of post-discharge clinical liabilities.
6. Tertiary Care Referral Letter
Formal transfer letter to higher specialized centers summarizing clinical history, investigations done, treatment administered, and reason for tertiary referral.
3. Generating & Printing a Document
- Navigate to Certificates > Generate Document.
- Select the patient UHID and choose the Certificate Template.
- The editor automatically merges patient data. The doctor can add specific remarks or edit text directly in the visual editor.
- Click Save & Print PDF. The document is saved permanently to the patient's record and can be reprinted anytime.
